What is a Supplemental Report?
A Supplemental Report is filed when there is a missing information in a person’s civil registry document.
Example:
- There is no place and date of marriage in a child’s Report of Birth
- The name of a parent is missing from a child’s Report of Birth
- The place of marriage is missing from a Report of Marriage
Under the Implementing Rules and Regulations of Philippine Civil Registration Laws, a supplemental report using the appropriate form may be filed to supply information inadvertently omitted when the document was registered.
Who may file a Supplemental Report?
The Supplemental Report may be filed by the parent/guardian or the party concerned, if of age, who shall execute an affidavit indicating the entry/ies missed in the registration and the reasons why there was a failure in supplying the required entry.
Restrictions on the filing of a Supplemental Report?
The filing of a supplemental report and the affidavit of explanation shall be subject to the following restrictions:
- The supplemental report shall not be used in any manner to change or to circumvent the provision of Article 412 of the Civil Code of the Philippines which prohibits any change or correction of an entry in the civil register without judicial order or through administrative correction under Republic Act No. 9048, as amended.
- The Consular Section shall accept only one (1) supplemental report for a maximum of two (2) omitted information in any registered event. In cases where there are more than two omitted information, all papers related thereto shall be forwarded to the Office of the Civil Registrar-General.
PROCEDURE FOR FILING A SUPPLEMENTAL REPORT
- Proceed to Window 2, Building 3, and submit all of the required documents
- The Consular Officer will review all documents and inform the client if there are discrepancies that need to be corrected or any missing document that needs to be presented.
- If all documents are complete in form and substance, the Consular Officer will direct the client to pay the fees at the Cashier.
- Client will proceed to the Cashier (1st Floor, Building 3) to pay the fees
- After payment, Client will return the receipt to the Consular Officer at Window 2.
- Once the fees are settled, the Consular Officer will inform the applicant of the next steps.

FEES FOR SUPPLEMENTAL REPORT
Notarization of the Supplemental Report: SR 100.00
Filipinos abroad may file a petition before the Philippine Consulate General in Jeddah to correct a clerical or typographical error in an entry and/or cause a change of First Name or Nickname in the Civil Register without need of a judicial order. This may be done under Republic Act 9048, as amended by Republic Act 10172.
What corrections can be made under RA 9048, as amended by R.A. 10172?
1. Correction of clerical or typographical errors in any entry in civil registry documents.
What is a clerical or typographical error?
It is an obvious mistake committed in clerical work, either in writing, copying, transcribing, or typing an entry in the civil register that is harmless and innocuous, such as the following:
- misspelled name or misspelled place of birth;
- mistake in the entry of day and month in the date of birth
- gender/sex (provided that this correction is not a result of a sex-reassignment)
The error must be visible to the eyes or obvious to the understanding, and can be corrected or changed only by reference to other existing record or records

2. Change of a person's first name or nickname in his/her civil registry document under the following grounds:
- The petitioner finds the first name or nickname to be ridiculous, tainted with dishonor or extremely difficult to write or pronounce.
- The new first name or nickname has been habitually and continuously used by the petitioner and he has been publicly known by that by that first name or nickname in the community: or
- The change will avoid confusion

What cannot be subject to administrative correction and must undergo judicial process for correction?
Those involving the change of
- Nationality,
- Age, or
- Status of the Petitioner (Legitimate/Illegitimate)
Who can file a Petition for Correction?
Someone who is of legal age (18 years old and above) and must have a direct and personal interest in the correction being sought.
If the record to be corrected belongs to a minor, an adult who has a direct and personal interest in the correction being sought must be the one to file the petition.
Who are considered to have a direct and personal interest in the correction being sought?

PROCEDURE TO REPORT CONVERSION TO ISLAM

The marriage between two (2) Filipinos or between a Filipino and a foreign national should be reported to and registered with the Philippine Statistics Authority (PSA) through the Philippine Embassy/Consulate General which has jurisdiction over the locality where the event took place.

- After payment, Client will return the receipt to the Consular Officer.
- Once the fees are settled, the Consular Officer will inform the applicant of when they can return to claim their signed and authenticated Report of Marriage Form.
