Requirements for the Renewal of Regular Passport:

1. Personal appearance
2. Original and photocopy of recent passport (data page only)
3. Passport application form that can be downloaded from here or obtained from Window 1 of the Consular Section (no photo needed). Make sure you complete all entries before you get to the encoders.
4. Fee: SR 240

Requirements for the Renewal of MRP or Green Passport:

1. Personal appearance
2. Actual Green passport and photocopy of data page
3. Birth Certificate issued by the Philippine Statistics Authority (NSO)
4. At least one (1) valid identification document (SSS ID, Philippine Driver's License, Voter's ID, PRC ID, etc.)
5. Other supporting document as necessary (i.e. Authenticated Marriage Certificate if married and using married surname
6. Passport application form that can be downloaded from here or obtained from Window 1 of the Consular Section (no photo needed). Make sure you complete all entries before you get to the encoders.

Requirements for the Replacement of Lost Passport:
(Requirements valid until 03 July 2013)

1. Personal appearance
2. Birth Certificate in PSA (formerly NSO) Security Paper
3. Affidavit of Loss
4. Police Report with English translation
5. Copy of Lost Passport, if available
6. Any ID or document to prove identity (needs verification from Manila)
7. Duly accomplished passport application form (no photo needed)

(Note: There is a 15 working days clearing period which will start upon submission of complete documentary requirements)

Requirements for the Renewal of Hajj / Umrah Passports:

1. Personal appearance
2. NSO copy of Local Civil Registrar Copy of the Birth Certificate duly authenticated by the DFA (Red Ribbon)
3. original and copy of Hajj / Umrah passport
4. Duly accomplished passport application form (no photo needed)
5. Fee: SR 240

Or

1. Personal appearance
2. Affidavit of two (2) disinterested persons
3. Any old document that provides the date of birth and place of birth (i.e. old NBI clearance, marriage contract, transcript of records, driver's license, postal ID, voter's ID, etc)
4. Certificate if no Record of Birth from the Local Civil Registrar of the place where the person was born
5. Duly accomplished passport application form (no photo needed)
6. Fee: SR 240

2. KUNG 6 MONTHS OR 1 YEAR ANG VALIDITY, KAILANGAN NG BIRTH CERTIFICATE PARA MAGPARENEW.

1. FILLED-UP PASSPORT APPLICATION FORM
2. KOPYA NG BIRTH CERTIFICATE
3. ORIGINAL AT KOPYA NG PASSPORT
4. BAYAD: SR240
5. KUNG 5 YEARS ANG VALIDITY, HINDI NA KAILANGAN NG BIRTH CERTIFICATE PARA MAGRENEW KAHIT GINAMIT SA UMRAH OR HAJJ

REQUIREMENTS: (SA PAGRENEW)

1. FILLED-UPPASSPORT APPLICATION
2. ORIGINAL AT KOPYA NG PASSPORT
3. BAYAD: SR240

Requirements for newly born applicants for e-Passport:

1. Report of Birth formreport of birth accomplished - six (6) copies.
2. Arabic Birth Certificate - six (6) copies + original
3. Translated Arabic Birth Certificate to English - six (6) copies + original
4. Birth Notification from the hospital - six (6) copies. + original
5. NSO Marriage Contract of parents duly authenticated by the Department of Foreign Affairs, Manila (if married in the Philippines) or authenticated by the Saudi Ministry of Foreign Affairs with English translation (if married in KSA) – two (2) copies + original
6. Photocopies of parents' passports (data page only) and iqama– two (2) copies
7. Duly accomplished passport application form (no photo needed)

• 30 days after applying for your passport, you may check the Embassy website at http://jeddahpcg.dfa.gov.ph/index.php/consular-services/passport/passports-available to find out if your passport has been received by the Embassy and is ready for release.  You may also call 482-3816.

Releasing of e-Passport

1. Releasing will be in the afternoon only from 1:00 pm to 4:00 pm.
2. Bring the following:
   1. Original old passport
   2. Official receipt

If you cannot appear personally to collect your new passport, you may authorize a representative and give him/her a signed letter of authorization stating your full name and the representative’s full name. 

Emergency Extension of Validity

 NOTE: This facility is allowed only to facilitate the travel of Philippine passport holders for a valid and urgent reason. Extension cannot be done twice. The passport holder must apply for passport renewal at the same time of the emergency extension.

Requirements: Original (and copy of) passport; duly accomplished form for extension of passport validity; affidavit explaining the reason for your application for extension; and any proof of emergency, as supporting document ( as appropriate).

 Consular Service Fee: SR80.00 (in addition to fees for passport renewal).
