The Philippine Consulate General would like to inform our Kabayans in Jizan and nearby areas that a team will be at the Adnan Hotel on 03 September 2016 from 8:00 a.m. to 5:00 p.m. to conduct Consular Mobile Services (CMS). The following services will be available during the CMS:
- Passport renewal and extension
- Notarials
- Endorsement for Police clearance
- Report of Birth
- Labor Assistance
- SSS services
- PAG-IBIG services
The Consulate would also like to remind passport renewal applicants that those still with Machine Readable Passports (passport number starting with “XX”) and“Green” Passports need to submit the following:
- Actual passport;
- Original Birth Certificate issued by the Philippine Statistics Authority (NSO);
- At least one (1) valid identification document (SSS ID, Philippine Driver’s License, Voter’s ID, PRC ID etc.)
- Other support document as necessary (i.e. Authenticated Marriage Certificate if married and using married surname).
